DeCoach Recovery Centre in Fairborn, OH is seeking a Case Manager to provide community-based support to adults in recovery. You will connect clients to resources, help build daily living skills, advocate for clients, and support recovery goals.

Flex hours may be available to serve client needs. Responsibilities include managing cases under clinical supervision, coordinating care, traveling in the community with a company vehicle, documenting services, and collaborating with the treatment team to

Qualificações

• High school diploma or college degree from an accredited institution.
• Active Ohio driver's license with a clean driving record.
• At least one year of experience in behavioral health or a related field.
• Excellent communication skills and working knowledge of behavioral health issues and current therapeutic methods.

Responsabilidades

• Provide individual and group case management under clinical supervision
• Support clients through community activities, facility visits, vocational training, and coordination of care, as outlined in each Individual Treatment Plan
• Travel to meet clients in the community using a company vehicle (properly insured)
• Document services accurately and follow all company policies and procedures
• Work with the treatment team and managers to improve the quality of our services
