Job summary

Community Empowerment Resources (CER) in Hawaii seeks a compassionate, recovery‑oriented Community-Based Case Manager to provide individualized support to adults living with serious mental illness in the community. You will build trusting relationships, coordinate services across behavioral health, housing, medical and social needs, and develop recovery plans.

Qualifications

• Valid driver’s license and reliable transportation.
• Combination of education and/or experience with vulnerable populations.
• Willingness to work in community-based settings.

Responsibilities

• Develop, implement, and update individualized service and recovery plans based on client goals, strengths, needs, and preferences.
• Build therapeutic, recovery-oriented relationships to promote engagement, stability, and independence.
• Coordinate access to psychiatric, medical, behavioral health, housing, legal, vocational, substance use, and other services.
• Provide home visits and community meetings to address client needs and progress toward recovery.
• Collaborate with treatment providers, families, and community partners to coordinate care and remove barriers.
• Provide transportation to appointments and accompany clients to resources when appropriate.
• Monitor progress and adjust service plans with clients and the treatment team.
• Maintain accurate, timely documentation per agency policies and funding requirements.
