Job summary

NorthCare seeks a Behavioral Health Case Manager II to deliver integrated services spanning physical and mental health, while engaging clients and families as partners in care. The role emphasizes trauma-informed, culturally competent, recovery-focused care within a multidisciplinary team.

Qualifications

• High school diploma or GED required.
• Minimum 36 months direct consumer care in mental health or substance use field.
• ODMHSAS Case Management II certification within 60 days of hire, if not currently certified.

Responsibilities

• Provides integrated services addressing physical health care, behavioral healthcare and social needs that are recovery focused and patient centered.
• Coordinates care across treatment settings and with community partners to support treatment goals.
• Assesses client symptoms, functioning, history and preferences to develop a care management plan.
• Assists clients with eligibility for benefits, housing, transportation, education or vocational programs, employment and service linkage.
