Qualifications

• LSW/LPC required, LISW/LPCC preferred.
• Three years of experience providing coordinated supportive services/care coordination services.

Responsibilities

• Supervise care coordination team members through team meetings and regular communications.
• Provide training for direct care coordinators in home and community settings.
• Ensure completion of required care plans and documentation.
