Job summary

Easterseals PORT Health is seeking a community-based nurse to provide hands-on behavioral health care in homes, shelters, and community settings across North Carolina. The role emphasizes medication administration, crisis intervention, and collaborative care with the team psychiatrist.

You will participate in ACT Team meetings, educate clients on health management, and advocate for access to community resources.

Responsibilities

• Administer and monitor prescribed psychotropic medications.
• Collaborate with the team psychiatrist on medication plans.
• Participate in daily ACT Team meetings for coordinated care.
• Provide crisis assessment and on-call interventions.
• Deliver nursing care in clients' homes and community settings.
• Educate individuals on health management, wellness, and medication adherence.
• Advocate for client choice and connect individuals with community resources.
• Strong communication, documentation, and relationship-building abilities.
