Job summary

Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. This role delivers community-based behavioral health care to adults in Durham, Person and Orange Counties, working outside traditional facilities.

You will provide psychotropic medication management, nursing assessments, and care coordination, collaborating with psychiatrists and therapists to support stability and improved quality of life in diverse community settings.

Qualifications

• Active, unrestricted RN license in North Carolina.
• Experience in behavioral health or mental health (preferred).
• Strong communication, documentation, and relationship-building abilities.

Responsibilities

• Administer and monitor psychotropic medications.
• Conduct nursing assessments and track symptoms, side effects, and medical issues.
• Document all care and observations in the Welligent EMR.
• Partner with the team psychiatrist to update medication protocols and care plans.
• Participate in daily in-person ACT Team meetings to ensure coordinated, person-centered treatment.
• Provide crisis assessment and intervention as part of a rotating on-call schedule.
• Deliver nursing care in clients’ homes and community settings.
• Educate individuals on health management, wellness, and medication adherence.
• Advocate for client choice and connect individuals with needed community resources.
