Job summary

Central Minnesota Mental Health Center is seeking a Behavioral Health Practitioner housing specialist to join our ACT team in St. Cloud. You will work in the community to help clients find and sustain housing, advocating with landlords and coordinating housing supports.

You’ll conduct outreach in clients’ homes and neighborhoods, document services, and collaborate with a multidisciplinary team to promote recovery and stability for individuals with serious mental illness.

Qualifications

• Qualify as a Behavioral Health Practitioner (BHP)
• 4,000 hours working with individuals with mental health, developmental disabilities, or chemical dependency diagnoses
• Bachelor’s degree in behavioral sciences or related field (or master’s degree) with practicum/internship or employment involving direct interaction with adults or children served
• Clear DHS background study
• Valid Driver’s License and acceptable driving record
• Experience providing housing support services or related field in community settings

Responsibilities

• Help people find and maintain a place to call home by assisting with housing searches, applications, subsidies, and other housing resources.
• Meet clients where they are, providing services in homes, neighborhoods, community settings, and other meaningful locations.
• Assess housing-related strengths, needs, preferences, and barriers and help clients develop skills for independent living.
• Advocate for clients to receive appropriate housing and supports while promoting independence and self-determination.
• Build relationships with landlords, property managers, housing providers, community agencies, families, and other supports.
• Navigate challenges that can affect housing, such as landlord concerns, lease issues, and changes in income or benefits.
• Support transitions between housing settings and assist with relocation when needed.
• Participate in treatment and discharge planning for clients in inpatient or Crisis Residential services.
• Maintain client files per Minnesota statute and document services accurately and timely.
• Collaborate with an interdisciplinary ACT team and other organizations to coordinate services.
