CMMHC has a rewarding opportunity to serve as a Behavioral Health Practitioner: Housing Specialist with our St. Cloud Assertive Community Treatment (ACT) team.
If you are looking for a position where you can make a difference beyond the walls of an office, ACT may be the right fit for you.
Assertive Community Treatment (ACT) is a unique, community-based model in which a multidisciplinary team brings services directly to the people we serve. Instead of expecting clients to come to an office for support, ACT team members meet people where they are—in their homes, neighborhoods, workplaces, and communities—and work alongside them to achieve their goals.
As the Housing Specialist, you will be an essential member of this team, helping people with serious and persistent mental illness find, obtain, and maintain housing that is safe, stable, and consistent with their personal goals. You will build relationships, problem‑solve alongside clients, advocate with landlords and community partners, and help clients develop the skills and supports they need to successfully live in the community.
This is not a typical office‑based housing position. You will spend your time in the community, working directly with clients and collaborating closely with an interdisciplinary team. You will have the opportunity to see the impact of your work in people's everyday lives.
