Central Minnesota Mental Health Center in St. Cloud, MN is expanding its Assertive Community Treatment (ACT) team with a Behavioral Health Practitioner housing specialist role.

This is a field‑based position delivering services to clients in homes, neighborhoods, and community settings to help them live safely and independently. You will partner with a multidisciplinary team, build relationships with landlords, assess housing needs, advocate for supports, and coordinate care across services.

Responsibilities

  • Help people find and maintain housing by assisting with searches, applications, subsidies, and other housing resources.
  • Meet clients where they are, providing services in homes, neighborhoods, community settings, and other locations that are meaningful to the client.
  • Assess housing-related strengths, needs, preferences, and barriers and help clients build the skills needed to obtain and maintain independent living.
  • Advocate for clients to receive the appropriate level of housing and supports while promoting independence, choice, and self‑determination.
  • Build relationships with landlords, property managers, housing providers, community agencies, families, and other natural supports.
  • Help clients navigate housing challenges such as landlord concerns, leases, income changes, and tenancy barriers.
  • Support clients through transitions between housing settings and assist with relocation when needed.
  • Participate in treatment planning and discharge planning for clients receiving inpatient or Crisis Residential services.
  • Provide care coordination within CMMHC and with external providers per CCBHC language.
  • Maintain knowledge of mental health, housing, medical, financial and social services and connect clients with resources.
  • Establish meaningful, trusting relationships with assigned clients and their supports and meet with each client as planned.
  • Participate actively in the ACT team's daily operations, including team meetings, consultation, problem‑solving, and coordination of services.
  • Complete responsibilities related to housing contracts and documentation.

Assertive Community Treatment (ACT) is a unique, community-based model in which a multidisciplinary team brings services directly to the people we serve. Instead of expecting clients to come to an office for support, ACT team members meet people where they are—in their homes, neighborhoods, workplaces, and communities—and work alongside them to achieve their goals.

As the Housing Specialist, you will be an essential member of this team, helping people with serious and persistent mental illness find, obtain, and maintain housing that is safe, stable, and consistent with their personal goals. You will build relationships, problem‑solve alongside clients, advocate with landlords and community partners, and help clients develop the skills and supports they need to successfully live in the community.

This is not a typical office‑based housing position. You will spend your time in the community, working directly with clients and collaborating closely with an interdisciplinary team. You will have the opportunity to see the impact of your work in people's everyday lives.

  • Participate in treatment planning and discharge planning for clients receiving inpatient or Crisis Residential services.
  • Provide care coordination within CMMHC and with outside organizations and providers, including entities identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination.
  • Maintain current knowledge of the mental health, housing, medical, financial, and social service systems and connect clients with resources that support their recovery and stability.
  • Establish meaningful, trusting relationships with assigned clients and their supports and meet with each client as planned.
  • Participate actively in the ACT team's daily operations, including team meetings, consultation, problem‑solving, and coordination of services.
