Job summary

Harbor, Inc. seeks a Behavioral Health Case Manager to join the Toledo team. The role focuses on community-based case management for adults with mental health challenges, helping access resources and improve daily functioning for better quality of life.

This full-time position requires coordination across providers, accurate documentation, and delivery of CPST/TBS with adherence to standards. Training and licensure requirements apply.

Qualifications

• Must have a Bachelor’s degree in counseling, social work, psychology, or a closely related behavioral/mental health field from an accredited college or university.
• High School Diploma or GED with 3 years of experience in a behavioral health or related field may be accepted in lieu of Bachelor’s degree.

Responsibilities

• Serves as primary clinician for assigned clients, coordinating care and updating ISP regularly.
• Completes documentation including progress notes, ISPs, needs assessments, discharge summaries, and other forms.
• Provides CPST and therapeutic behavioral services, including Day Treatment groups, per standards.
