Qualifications

• Bachelor's degree in related field.
• Valid Driver's License with clean driving record.
• Ability to obtain NPI Number and eligibility to apply for Ohio Medicaid Number.

Responsibilities

• Driving to client residences and other locations in the community with client.
• Actively engaging with individuals and delivering client-centered care in the community.
• Meeting weekly client care hours by spending 80% of your time in the community with clients.
• Conducting thorough assessments of clients' needs and developing Individualized Service Plans.
• Coordinating and advocating for needed services, including healthcare, housing, and social support.
• Collaborating with community resources and providers to ensure clients receive appropriate care.
• Monitoring and evaluating clients' progress and adjusting Individual Service Plan as needed.
• Providing support and crisis intervention as necessary.
