Coleman Health Services in Stark County, Ohio, is hiring an experienced Case Manager to join our Assertive Community Treatment (ACT) team. You will provide community-based outreach and support to individuals with severe and persistent mental illness, helping them access housing, services, and essential resources.

The role requires a BA/BS in mental health or related field, an excellent driving record, and flexibility for after-hours on-call.

Formación

• BA/BS in mental health or related field
• Obtain an NPI and an Ohio Medicaid number
• Maintain an excellent driving record
• Must be able to work a flexible schedule including after-hours on-call rotation

Responsabilidades

• Conduct assertive outreach in the community to individuals experiencing severe and persistent mental illness (SPMI).
• Engage and build relationships with individuals who are most at risk.
• Assist clients with accessing community-based resources and support services.
• Help clients obtain and maintain stable housing and employment.
• Coordinate appropriate referrals to community providers and other supportive services.
• Assist clients with scheduling, accessing, and maintaining appointments.
• Collaborate with a multidisciplinary team to support client recovery, independence, and overall well-being.
