Job summary

Qualifications

• Experience with SPMI and recovery-oriented supports is desirable.

Responsibilities

• Assist patients in identifying strengths to promote recovery and independence.
• Provide engagement, vocational assessment, job development, placement and follow-along supports.
• Educate ACT patients on disability benefits programs as applicable.
• Develop relationships with local businesses for employment opportunities.
• Refer patients to social services and provide case management and supports.
