Job summary

InterCommunity Health Care seeks an ACT Community Support Specialist to join the Assertive Community Treatment Team, providing care coordination in clients’ homes and community settings. You will work on a transdisciplinary team, linking clients to clinical, medical, social, and rehabilitative services and supporting recovery through individualized treatment sessions.

• Bachelor's or associate degree in health/behavioral health.
• Experience in community health or behavioral health settings preferred.
• Experience assisting recovery from mental illness and substance use.
• Ability to link clients to community resources.

Responsibilities

• Active participation on a transdisciplinary ACTT team with daily meetings.
• Assist in linking each client to clinical, medical, social, educational, rehabilitative or vocational services.
• Collaborate to identify required services based on client needs.
• Support client recovery through treatment sessions within the ACT Team or outside providers.
• Complete documentation including court letters, medical records, and scheduling.
