Job summary

Hope Haven of Northeast Georgia Inc. is seeking a Community Access Group (CAG) Program Coordinator to oversee community-based services for adults with disabilities. The role emphasizes supervision of staff, program compliance, and coordination of case management across all CAG activities.

You will ensure services meet regulatory requirements, develop resources for persons served and families, and foster community integration while maximizing billing and maintaining strong community partnerships.

Qualifications

• B.S. or B.A. in Psychology, Sociology, Social Work, Education, or closely related field preferred.
• Supervisory experience.
• Experience with IDD population or specialized training.
• Valid Driver’s License and Vehicle Liability Insurance.
• Acceptable Motor Vehicle and Criminal Records reports.
• Pre-Employment Drug Screen
• TB screen
• Office Hours: Monday-Friday 8am-4pm

Responsibilities

• Oversee and manage the Community Access Group (CAG) services and supervise staff.
• Ensure program components meet Federal, DBHDD, CARF, and agency requirements.
• Coordinate case management and records for all persons served in CAG programs.
• Develop and implement community integration activities and volunteer opportunities.
• Monitor program outcomes and participate in CQI-RM activities.
• Perform administrative tasks and represent the program with partner agencies.
