Job summary

Great Bay Staffing Group is seeking a dedicated Physical Therapist for a full-time Home Health role in Colorado. You will assess patients, design and implement individualized treatment plans, and guide patients and families through recovery and home exercise programs.

You will collaborate with a multidisciplinary team to ensure coordinated care, document progress, and maintain high professional standards while advancing your clinical skills in a supportive home health setting.

Qualifications

• Master's degree in Physical Therapy from a CAPTE-accredited program.
• Minimum of nine (9) months of professional experience as a licensed Physical Therapist.
• Colorado license in good standing (or ability to obtain).
• CPR/BLS certification required and must be maintained throughout employment.
• Valid driver's license with reliable transportation to travel to patient homes and business locations.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities for patient education and team collaboration.
• Commitment to maintaining professional standards and continuing education in physical therapy practice.

Responsibilities

• Conduct comprehensive physical therapy assessments and evaluate patients' medical histories to determine appropriate treatment approaches
• Develop and implement customized treatment plans designed to meet individual rehabilitation goals and patient-specific needs
• Deliver therapeutic exercises, manual therapy techniques, and functional training programs to improve patient mobility, strength, and overall functionality
• Provide patient and family education on injury prevention strategies, wellness practices, home exercise programs, and the rehabilitation process
• Monitor, evaluate, and document patient progress throughout treatment, adjusting rehabilitation plans as needed to optimize outcomes
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to ensure comprehensive, coordinated patient care
• Maintain accurate and detailed patient records, clinical documentation, and progress notes in compliance with regulatory standards and healthcare guidelines
