Job summary

Benefit Home Health is seeking a licensed Physical Therapist to provide patient-centered PT services in home health settings across Colorado. You will develop individualized care plans, collaborate with physicians, and educate patients and families while ensuring high-quality outcomes.

Colorado licensure, at least 2 years of experience, CPR certification, and reliable transportation are required. The role offers a collaborative team environment and opportunities to contribute to discharge

Qualifications

• Licensed to practice as a physical therapist within the State of Colorado.
• Two years of experience as a physical therapist; home health experience preferred.
• Good verbal and written communication and strong organizational skills.
• Must hold CPR certification and have a valid driver’s license with access to a vehicle.

Responsibilities

• Provide physical therapy services per physician’s plan of care.
• Develop and monitor individualized plans of care in partnership with patient and caregivers.
• Counsel patients, caregivers, and families; document progress and notes.
• Coordinate with physicians and other health care practitioners on the home health plan of care.
• Instruct patients and families in therapy techniques and device usage.
• Participate in discharge planning and quality improvement activities.
• Supervise aides and collaborate with the interdisciplinary team.
