MultiCare Auburn Medical Center in Auburn, WA, is seeking a Licensed Integrated Mental Health Therapist (IMHT) to join a multidisciplinary primary care team using the Collaborative Care model. You will assess, treat, and plan care for diverse patients within a prevention and chronic disease management context.

This 1.0 FTE role features a Monday–Friday schedule from 8:00 AM to 5:00 PM, with potential up to a $5,000 sign-on and up to $10,000 relocation bonus.

Qualifications

• Master's degree in Social Work, Counseling, Marriage & Family Therapy, or related field.
• Mental Health Professional credentials: LMFT, LMHC, LICSW per HB 1724.
• Minimum two years of experience in a behavioral health setting.

Responsabilités

• Assesses patient symptoms and creates customized treatment plans.
• Screens for behavioral health disorders using PHQ9, GAD7, or related tools.
• Monitors symptoms and treatment side effects, aiming for improvement.
• Supports psychotropic medication management with PCPs.
• Tracks outcomes and re-engages patients using a registry.
• Facilitates care plan changes and referrals as needed.
• Manages behavioral health crises within a primary care setting.
• Provides flexible services including warm handoffs during medical visits.
