You will provide direct primary care across all ages, formulate diagnostic plans, interpret tests, and promote preventive care. You’ll collaborate with medical directors, behavioral health staff, nurses, and support teams, while maintaining precise EHR documentation.

Qualifications

• Active, unrestricted California RN and NP/NPF licenses.
• Current FNP board certification (AANP or ANCC) or NCCPA board certification for PA.
• Valid DEA license and BLS certification.
• 1+ year outpatient primary care experience; new graduates welcome.
• Experience in Community Health Centers / FQHC or serving underserved populations preferred.
• Bilingual in English and Russian/Spanish/Farsi/Pashto is a strong plus.

Responsibilities

• Provide direct primary care, routine screenings, and treatment for acute and chronic conditions across all age groups.
• Formulate diagnostic plans, interpret lab/diagnostic tests, and prescribe therapies per state guidelines.
• Promote preventive care and patient health education tailored to community needs.

Key Responsibilities

• Provide direct primary care, routine screenings, and treatment for acute and chronic conditions across all age groups.
• Formulate diagnostic plans, interpret lab/diagnostic tests, and prescribe pharmacologic therapies in alignment with state practice guidelines.
• Promote preventive care, wellness, and patient health education tailored to community needs.
• Collaborate closely with integrated health teams, including medical directors, behavioral health providers, nurses, and medical support staff.
• Maintain accurate, timely clinical documentation in the Electronic Health Record (EHR) system.

Qualifications & Requirements

• For Nurse Practitioners (NP): Active, unrestricted California RN and NP/NPF license.

• For Physician Assistants (PA): Active, unrestricted California Physician Assistant license.

Experience & Preferred Skills

• 1+ years of outpatient primary care or ambulatory clinical experience (new graduates with strong clinical rotations in community health or primary care are welcome to apply).
• Prior experience in a Community Health Center, FQHC, or serving vulnerable/underserved patient populations is strongly preferred.
• Bilingual proficiency in English and one of the following languages is a strong plus: Russian, Spanish, Farsi, or Pashto.
