Qualifications

• Certified Nurse Assistant (CNA) required.
• Must possess a valid state driver’s license.
• Clean driving and criminal records; comply with state certifications as required.

Responsibilities

• Assure resident safety during transport to appointments.
• Accompany residents to medical visits and stay with them until completion.
• Sign in residents and present documents at visits.
• Obtain documents, prescriptions, and ensure orders are correctly written before departure.
• Return to facility and report on visit details to the RCM.
• Perform other duties as assigned.
