Responsibilities

• Safely transport residents to and from medical appointments and outings.
• Assist residents with transfers in and out of the facility and vehicle.
• Remain with residents during appointments as needed and ensure safe return.
• Coordinate transportation schedules with nursing staff and providers.
• Communicate updates about resident needs and schedule changes.
• Perform pre-trip vehicle inspections and report safety concerns.
• Maintain cleanliness and safety of transportation vehicles.
• Follow facility policies, state regulations, and infection control guidelines.
• Provide supportive care consistent with CNA training.
