Job summary

Atrium Living Centers Inc. is seeking a Transport Driver/CNA in Wisconsin for the first shift. You will transport residents to appointments, assist during transport, and split duties with on-floor CNA tasks. A valid CNA license, clean driving record, and 25+ years old are required.

Qualifications

• Must be 25 years of age or older.
• Clean driving record; high school diploma or GED.

Responsibilities

• Schedule and/or provide transportation for residents to appointments.
• Assist residents safely during transport and appointments.
• Drive center van and transport up to two patients at a time.
• Split duties 50/50 between CNA responsibilities and transport.
• Provide resident care per approved Care Plan and assist daily activities.
• Maintain resident medical records and follow infection control protocols.
• Complete documentation including vitals and report rights violations.

What You’ll Do

• Schedule and/or provide transportation for residents to and from appointments

• Assist residents safely during transport and appointments

• Drive center van (similar in size to a minivan)

• Transport a maximum of two patients at a time

• Split duties 50/50 between floor CNA responsibilities and transport driving

• Provide resident care according to an approved Care Plan

• Assist with daily activities, personal care, and resident safety

• Maintain and respond to resident medical record requests

• Follow infection control procedures and safety protocols

• Complete required documentation, including vitals

• Report any violations of the Residents’ Bill of Rights immediately

What You Bring

• Completed CNA course and active state license

• Must be 25 years of age or older

• Clean driving record

• High school diploma or GED

• Medical Records experience in a long-term care setting (preferred)

• ART certification (preferred)

• Strong written and verbal communication skills

• Service-oriented mindset with a willingness to learn

• Positive, can‑do attitude and excellent attendance
