Job summary

Bend Transitional Care in Bend, Oregon is seeking a Full-Time Certified Nursing Assistant (CNA I) to join our nursing team. You will provide compassionate daily living assistance, mobility support, vital signs, and nutritional help to residents in a 60-bed skilled facility.

Ideal candidates have a high school diploma, Oregon CNA certification, CPR, and a willingness to work closely with a tight-knit team to ensure quality resident care and smooth transitions home or to the next level of care.

Qualifications

  • High school diploma or equivalent is required.

Responsibilities

  • Providing assistance with activities of daily living, including bathing, dressing, grooming, and personal hygiene
  • Assisting residents with mobility, transfers, and range-of-motion exercises
  • Taking and documenting vital signs, food and fluid intake, and output as directed
  • Assisting residents during meals and supporting their nutritional needs
  • Observing residents and promptly communicating changes in condition to the nursing team
  • Maintaining accurate and timely documentation
  • Building positive, respectful relationships with residents, families, and team members
  • Working collaboratively with the nursing team to provide exceptional resident care

We provide compassionate transitional and post-hospital care to help our residents regain their strength, independence, and confidence as they work toward returning home or transitioning to their next stage of care. Requirements
Education:
  • High school diploma or equivalent required.
Licenses/Certification:
  • Valid certification as a CNA in Oregon
  • Valid CPR certificate
Experience:
  • Six months experience in a long term-care environment (preferred but not required)
