Job summary

Transitional Care Management is seeking a Certified Nursing Assistant (CNA) at Frankfort Terrace to provide compassionate, one-on-one care to residents in a slower-paced intermediate care setting. You will monitor residents’ well-being, assist with daily activities, and help escort or accompany residents on outings while building meaningful connections.

Located in Frankfort, IL, this role offers flexible scheduling within 8-hour shifts, with day and overnight options.

Qualifications

• Must have CNA certification in IL.
• Prior experience or desire to work with behavioral population is helpful, but not required.
• New grads welcome to apply.

Responsibilities

• Provide compassionate care monitoring to residents.
• Assist and attend activity outings with residents and help with daily tasks (select clothing, set appointments).
• Engage with residents one-on-one to build relationships and ensure comfort in a slower-paced environment.
