Qualifications

Responsibilities

• Monitor and report changes in residents' condition to nursing staff.
• Assist with transfers, ambulation, positioning, and mobility needs.
• Maintain accurate documentation as required.
• Keep resident rooms and care areas clean, safe, and organized.
• Follow individualized care plans and facility policies.
