Job summary

Central Queens Rehabilitation and Nursing Center is seeking a Certified Nursing Assistant (CNA) to support residents with daily living activities, monitoring health and providing essential care. This role involves teamwork with nursing staff, accurate documentation in SigmaCare, and maintaining a safe, clean environment to ensure high-quality resident care.

The position requires NY CNA certification, good communication skills, and flexibility to cover various shifts.

Qualifications

• Current and valid NY CNA certification.
• Experience in caregiving or patient care settings desirable.
• Familiarity with SigmaCare EHR is a plus.

Responsibilities

• Assist Residents with daily living activities, including meals, turning, ambulation, and providing fluids and nourishment.
• Assist with bathing, dressing, grooming and personal hygiene.
• Observe residents and report health issues to supervising nurse.
• Monitor vital signs and record results (temperature, pulse, respiration, blood pressure).
• Provide basic life support as needed and respond to emergencies.
• Maintain infection control and a clean, safe environment.
• Assist nursing staff with procedures and documentation in SigmaCare.
• Document resident food and fluid intake and assist with feeding as needed.
• Make beds and keep living areas tidy; transport residents within the facility.
• Adhere to policies, confidentiality, and professional standards; support continuity of care.
• Communicate effectively with patients, families, and healthcare team; participate in training.

Certified Nursing Assistant (CNA) Duties and Responsibilities:

• Assist Residents with daily living activities, to include, delivering meals, turning and ambulating residents and providing plenty of fluids and nourishment meals throughout scheduled shift
• Assist patients with daily living activities such as bathing, dressing, grooming
• Help Residents with personal hygiene
• Observe Residents and report any health issues to the supervising nurse
• Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure
• Provide basic life support as needed and respond promptly to patient emergencies.
• Maintain a clean and safe environment for patients by adhering to infection control protocols
• Assist nursing staff with medical procedures and documentation using electronic health record systems such as SigmaCare
• Document Resident food and fluid intake
• Assist Residents with feeding and serving nutritional supplements
• Making the Resident's beds and keeping their living areas clean and tidy
• Transport Residents within the facility
• Assist nurses and other staff as needed
• Adhere to professional standards and follow all facility policies and procedures
• Abide by all federal, state and local requirements and standards
• Maintain the confidentiality of all Residents and Facility data
• Communicate effectively with patients, families, and healthcare team members to ensure continuity of care
• Participate in training sessions and continuing education to enhance skills and knowledge in patient care

Certified Nursing Assistant (CNA) Qualifications and Skills:

• Current and Valid NY CNA certification
• Experience in caregiving or patient care settings is highly desirable
• Familiarity with electronic health record systems SigmaCare is a plus
• Ability to work effectively within a team environment while also demonstrating independence when necessary
