Job summary

Chapters Health System in Cape Coral, FL is seeking a Certified Nursing Assistant to provide personal care and home making services for PACE participants in the community. You will follow instructions from the primary nurse and perform ADL/IADL assistance, mobility support, and ROM guidance to maintain safety and comfort.

You will document observations for the nurse and IDG, coordinate with families, and adapt to local coverage needs.

Qualifications

• High school diploma or GED, CNA training certificate.
• Florida CNA license is active and in good standing.
• CPR certification required and BLS preferred.
• Must pass competency evaluation and background check.
• Valid Florida driver's license with clean record.

Responsibilities

• Provide personal care and hygiene to patients.
• Assist with ADLs, IADLs, mobility, and ROM.
• Follow nurse instructions and document care provided.
• Coordinate with families and primary nurse for care plans.
• Adhere to HIPAA and maintain confidentiality.
