Job summary

Avenues Home Care is seeking a CNA Nurse Aide to support elderly and vulnerable clients in Brentwood, Nashville and surrounding areas. You will perform ADL assistance, medication help, light housekeeping, meal prep, and companionship according to each client’s plan of care.

You must follow care plans, report daily activities, maintain confidentiality, and uphold safety. We offer flexible shifts (full-time, part-time, 8/10/12 hour, PRN) with benefits like health, dental, vision coverage, mileage

Qualifications

• Must have good communication skills.
• Ability to follow verbal and written assignments.
• Proficiency with ClearCare Homecare Management System.
• CNA license or caregiver experience preferred.
• Reliable transportation.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) as specified in the client’s plan of care.
• Assist with medication, housekeeping, laundry, meal preparation, feeding, shopping, transportation and companionship.
• Report daily activity to your supervisor through the agency management system and logs.
• Other household tasks as specified in the client’s plan of care.
• Maintain confidentiality of records and ensure HIPAA compliance.
