Job summary

Avenues Home Care in Nashville, TN is seeking a Certified Nursing Assistant (CNA) to support elderly and disabled clients in the local area, including Gallatin and surrounding communities. The role emphasizes safety, reliability and compassionate, respectful care aligned with each client’s plan of care.

You will assist with ADLs, provide basic medical support, accompany clients to appointments, and complete daily logs for supervisor reporting.

Qualifications

• Excellent communication skills for coordinating with clients, families and supervisors.
• Ability to follow verbal and written care plans precisely.
• Proven patient care abilities with a compassionate approach.

Responsibilities

• Assist with Activities of Daily Living (ADLs) as defined in the plan of care.
• Provide medication assistance, housekeeping, laundry, meals and transportation.
• Report daily activity to supervisor via agency system and logs.
• Maintain safety and confidentiality in all client interactions.
