Job summary

Ebenezer Ridges Care Center in Burnsville, MN seeks a Certified Nursing Assistant (CNA/NAR) to provide hands-on resident care in a skilled nursing setting. The role supports long- and transitional care, with evening shifts and every-other-weekend coverage, offering comprehensive benefits.

Responsibilities include personal cares, vitals, charting in EMR, and timely communication with the team. Active MN CNA registry by start date or enrollment in Ebenezer training is required.

Qualifications

• Active Nursing Assistant (NAR) on MDH registry by start date or enrolled in Ebenezer CNA pathway with 4-month post hire to registry.
• Ability to provide compassionate, patient-centered care to geriatric residents.
• Experience in diverse healthcare settings and teamwork.

Responsibilities

• Assist residents with personal cares per care plan (bathing, hygiene, dressing).
• Record vitals and observe resident status; report to staff promptly.
• AM/PM/NOC care including peri care and catheter care.
• Transfer residents safely with assistive devices as needed.
• Document in EMR and respond to call lights; support family inquiries.

We are looking for a Certified Nursing Assistant (CNA/NAR) to support our residents in our Skilled Nursing Facility- Ebenezer Ridges Care Center! This role provides hands on care and support services for our senior residents at our spacious care center (transitional care and long term care) in Burnsville, MN. This patient support position includes;

Responsibilities

• Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights
• Nursing Assistant may per community patient care model check and record vitals
• AM/PM/NOC cares; including peri care and catheter care
• May turn, lift or transfer residents with or without other persons/mechanical assistance
• Observes residents and reports status to appropriate staff in a clear and concise manner
• Chart or document in electronic medical record system
• Seeks to resolve resident and family concerns
• Receives and gives report to appropriate staff during various times in work shift
• Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner

Required:

• Active as Nursing Assistant, Registered (NAR) on the MDH Nursing Assistant Registry by start date – OR- Individual who is hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.)

Preferred:

• Ability to work with a diverse and geriatric population
• Communicate efficiently, with compassion and understanding for residents and coworkers
• Experience working as a HHA, PCA, DSP, NA, Resident Assistant
• Work experience in a customer service setting
