Job summary

New York Cancer & Blood Specialists (NYCBS) in Bohemia, NY is looking for a CNA/Medical Assistant to join the Chronic Care Management team. You will introduce the CCM program to eligible patients by phone, obtain verbal consent, and document in the EMR.

Under supervision, you will review and monitor documents related to the patient’s individualized care plan, advocate for patients, coordinate with wellness services, and ensure timely information flow between providers and patients.

Qualifications

• High School Diploma or equivalent.
• Certified CNA, or Medical Assistant.
• 2+ years of applicable experience.
• Bilingual, English/Spanish a plus.
• Proficiency in Word/Excel.

Responsibilities

• Work within the Chronic Care Management team.
• Introduce the CCM program to eligible patients via telephone, obtain verbal consent and document in the EMR.
• Review and monitor documents related to the patient’s individualized care plan under supervision.
• Advocate for patients and coordinate with wellness services at NYCBS.
• Facilitate communication between providers and patients and ensure access to services.
