Job summary

New York Cancer & Blood Specialists is seeking a Registered Triage/CCM Nurse (RN) to join our team in Bohemia, NY. This role blends triage and chronic care management to support patients across the practice.

You will perform telephonic symptom assessments, educate patients, create care plans, and collaborate with physicians, APPs, and staff while maintaining thorough documentation and adherence to HIPAA and clinical protocols.

Qualifications

• Active and unrestricted RN license.
• Excellent communication skills, both verbal and written.
• Proficient in using electronic health record systems.

Responsibilities

• Share responsibilities between Triage and Chronic Care Management.
• Perform thorough telephonic symptom assessment for patients practice-wide.
• Utilize evidence-based triage protocols to determine appropriate level of care and urgency while providing clinical decision-making within scope of practice.
• Deliver patient education regarding symptom management, treatment side effects, medication guidance and support.
• Responsible for the creation of care plans for newly enrolled patients; revise as clinically appropriate.
• Coordinate care with physicians, APPs, and ancillary staff as needed.
• Maintain detailed documentation compliant with healthcare regulations and standards.
• Maintain adherence to clinical protocols, safety standards, HIPAA regulations, and practice policies.
• Provide exceptional customer service while maintaining professionalism in high-volume call environments.
• Establish and revise patient- and disease-centered comprehensive care plans with measurable goals.
• Assume additional duties as assigned by management.

Job Description

Registered Triage/CCM Nurse (RN)

Location: Bohemia, NY

Hours/Days: 5/8.5 hour days, Mon-Fri 7:30a-6:30p

Duties And Responsibilities

• Share responsibilities between Triage and Chronic Care Management
• Perform thorough telephonic symptom assessment for patients practice- wide
• Utilize evidence-based triage protocols to determine appropriate level of care and urgency while providing clinical decision-making within scope of practice.
• Deliver patient education regarding symptom management, treatment side effects, medication guidance and support.
• Responsible for the creation of care plans for newly enrolled patients. They will also be responsible for revising the care plan as clinically appropriate to maintain relevance and accuracy.
• Coordinate care with physicians, APPs, and ancillary staff as needed.
• Maintain detailed and accurate documentation, utilizing appropriate focused assessment documentation in compliance with healthcare regulations and standards.
• Maintain strict adherence to clinical protocols, safety standards, HIPPA regulations, and practice policies.
• Provide exceptional customer service while maintaining professionalism, empathy, and efficiency in high-volume call environments.
• Establish, implement, and continuously revise patient- and disease-centered comprehensive care plans
• Define measurable treatment goals, identify potential barriers to care, and outline targeted intervention to drive positive patient outcomes.
• Assumes additional duties and responsibilities as assigned by the management team, consistent with organizational needs and departmental objectives.

Qualifications

• Excellent communication skills, both verbal and written
• Proficient in using electronic health record systems
