Job summary

Santè in Mesa, AZ is seeking a Certified Nursing Assistant to provide compassionate personal care in patients' residences under the supervision of a registered nurse, following evidence-based practices and fostering holistic, patient-centered care.

Responsibilities include assisting with admissions, transfers and discharges, and daily care plan reviews to ensure wishes are met; CPR/fingerprint clearance and CNA licensure are required, with six months of CNA/LNA experience preferred.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience.
• LTC/SNF/Rehab preferred.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
