Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/SNF or Transitional Rehab experience.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patient's routine to ensure personal care needs are met per the care plan.
• Review care plans daily to determine if changes in the guest/patient's daily care routine have been made on the care plan.
