Alumus Healthcare in Chandler, AZ seeks a Certified Nursing Assistant to provide dedicated personal care for patients in their homes under the supervision of a registered nurse. The role focuses on maintaining patient wellbeing, ensuring adherence to the care plan, and supporting admissions, transfers, and discharges.

The ideal candidate holds a valid CNA license, current CPR and fingerprint clearance, and has 6 months of CNA/LNA experience, preferably in LTC/SNF or Transitional Rehab settings.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience
• LTC/SNF/Rehab preferred.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
