Job summary

Santè is hiring a Certified Nursing Assistant to provide personal care in the patient’s residence under RN supervision. You will assist during admissions, transfers, and discharges and ensure care plans are followed. CPR and fingerprint clearance are required, with at least six months of CNA/LNA experience preferred.

Qualifications

• CNA license/certification required prior to hiring.
• Current CPR and fingerprint clearance card required.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care / Skilled Nursing Facility or Transitional Rehab experience preferred.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patient's personal care needs in accordance with the care plan.
• Review care plans daily to determine if changes in the daily care routine have been made on the care plan.
