Job summary

The Grove Post Acute in Woodland, CA is seeking a committed Certified Nursing Assistant to support residents with daily activities and nursing care per the care plan. You will use wristbands/ photos for identification, document ADLs and vitals, and promptly report changes to the Charge Nurse.

Qualifications

• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• National Provider BLS or CPR required upon hire.

Responsibilities

• Assist residents with routine daily activities and nursing care per the resident's care plan.
• Identify residents using wristband or photo before treatments, meals, etc.
• Record ADLs, I&Os, meals intake, vitals, bowel and bladder on flow sheets.
• Report all changes in resident condition to the Nurse Supervisor/Charge Nurse.
• Transport residents to activities and monitor safety.
• Assist in passing meal trays and ensure correct tray for each resident.
• Support and participate in a hydration program.
• Provide compassionate bedside assistance during meals.
