Job summary

Sonoma Post Acute in Sonoma, CA is seeking compassionate Certified Nursing Assistants (CNAs) to join our growing team. We provide high-quality skilled nursing and rehabilitation services in a supportive, team-oriented environment where residents come first.

You will assist residents with ADLs, take vital signs, help with meals, and document care while following safety and HIPAA guidelines. Multiple shifts are available, including full-time, part-time, and PRN, with competitive pay.

Qualifications

• Active California CNA certification required.
• Compassionate, dependable, and team-oriented attitude.
• Strong communication and observation skills.
• Ability to follow care plans and document accurately.

Responsibilities

• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility.
• Take and record vital signs and report changes promptly.
• Assist with meals, hydration, feeding, and after-meal care.
• Maintain a clean, safe, and comfortable resident environment.
• Respond to call lights and resident needs quickly and compassionately.
• Assist with admissions, transfers, and discharges.
• Document care provided in charts and flow sheets.
• Follow infection control, safety, HIPAA, and privacy procedures.
• Work closely with nurses and interdisciplinary team members.
• Support restorative nursing and rehabilitation programs.
