Job summary

Frontier Hospice is seeking a Per Diem Certified Nursing Assistant (CNA) to provide hands-on end-of-life care in patients' homes in Edmond, Oklahoma. You will work closely with our nursing team to meet physical, emotional, and personal needs with compassion and professionalism.

Requirements include a valid CNA certificate, CPR certification if required, a high school diploma or GED, and a valid driver’s license with reliable transportation.

Responsibilities

• Provide personalized care following the plan of care to ensure patient comfort and well‑being.

• Support families by sharing knowledge and guidance in caregiving for terminally ill loved ones.
• Maintain accurate documentation of care provided in the patient home and agency records.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver’s license, vehicle insurance, and access to dependable transportation.

$17.50 - $20.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
