Qualifications

As part of our interdisciplinary Home Care team, our CNAs (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting. We serve patients throughout Milwaukee County.

As a Home Health Aide in Home Care, you will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment, and contribute to a healthy environment for patients. These activities are provided under the supervision of a Registered Nurse or Therapist, following physicians' orders.

  • Support patient care by reviewing care plans prior to providing care. Notify the Case Manager of issues or concerns regarding the care plan and when revisions are needed.
  • Assist with daily living activities such as bathing, dressing, transfers, range of motion exercises, toileting, etc., under the supervision of the Case Manager.
  • Explain the care being provided to patients and involve them in their cares.
  • Complete mobile care tasks upon arrival and departure from patients' homes, in accordance with agency policy. This includes documenting mileage logs accurately and timely.
  • Work cooperatively with patients, care providers, family members, and agency staff by listening, responding to needs, communicating effectively, and reporting pertinent information to the Case Manager promptly.
  • Perform home maintenance tasks as per the care plan, such as house cleaning, laundry, meal preparation, and transportation.
