As part of our interdisciplinary Home Care team, our CNAs (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting. We serve patients throughout Milwaukee County.

As a Home Health Aide in Home Care, you will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment for patients, and contribute to a healthy environment for patients. These activities are provided under the direction and supervision of a Registered Nurse or Therapist, in accordance with the physicians' orders.

  1. Support patient care by reviewing care plan in the home prior to providing cares. Notify Case Manager with issues/concerns regarding care plan and when cares need revision.
  2. Provide assistance with daily living under the supervision of the Case Manager, e.g., bathing, dressing, transfers, range of motion, toileting, etc.
  3. Explain to patients the care being provided and involve patients in cares.
  4. Consistently complete mobile care upon arrival and departure from patients' homes per agency policy. Mobile care includes tasks completed in accordance with the care plan. Complete mileage logs timely and accurately.
  5. Work cooperatively with patients, care providers, family members, and agency staff by listening and responding to patient needs, engaging in appropriate communication, and reporting pertinent patient information to the Case Manager in a timely manner.
  6. Regularly complete home maintenance per plan of care, e.g., house cleaning, washing dishes, laundry, meal preparation, transportation, etc.

  • Prior experience working with the public is required.
  • Current/valid WI driver's license; reliable/insured vehicle.

Part-time, 28 hours/week, 8am-4:30pm, including weekend and holiday rotation.

Hourly, non-exempt position.
