Job summary

Action Health Staffing is seeking a Certified Nursing Assistant for home care, providing in-home patient care and support to clients in their residence. This role emphasizes compassion, reliability, and adherence to the Plan of Care.

Candidates must maintain DFS certification, have a high school diploma or GED, and demonstrate strong communication, physical endurance, and the ability to follow directions. Travel within the service area is required.

Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Possess effective communication skills to read, write and follow directions.
• Ability to perform physical tasks such as lifting, bending and standing.
• Must have sympathetic attitude toward the care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Provide personal hygiene care (bath, shave, hair care, mouth care).
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as instructed.
• Prepare meals with client preferences and diets.
• Provide companionship and activities based on client needs.
• Light housekeeping duties as assigned.
• Maintain complete and accurate records required by agency.
• Record changes in client's condition and report them.
• Follow Plan of Care and EVV time sheets as needed.
