A Place at Home - Kansas City South is seeking a dedicated Home Care Coordinator/CNA to join our team. You will schedule and coordinate caregiver assignments, provide direct care when needed, and build strong relationships with caregivers and clients in the Kansas City South area.

The ideal candidate has a valid CNA certificate, at least 2 years of caregiver scheduling or care coordination experience, and excellent organizational and communication skills to support seniors and their families.

Qualifications

• CNA certification in KS or MO, as applicable.
• Minimum 2 years experience in caregiver scheduling or care coordination.
• Experience caring for seniors.
• Strong organizational and communication skills.
• Willingness to travel to client homes.

Responsibilities

• Manage and maintain caregiver schedules for client coverage.
• Match caregivers to clients based on skills and needs.
• Address scheduling gaps and conflicts.
• Provide direct non-medical personal care when needed.
• Assist field staff with client routines and mobility.
• Conduct caregiver introductions and visits.
• Build positive relationships with caregivers, clients, and families.
