Prisma Health is seeking a Certified Nursing Assistant to provide direct participant care at the SeniorCare PACE-Laurel center in a day shift. You will follow an individualized plan of care, document patient status, and support treatments under the Interdisciplinary Team.

The role requires CNA or Nursing Assistant/PCT program completion and at least one year of experience with frail elderly individuals; state CNA certification may be accepted in lieu of verification at hire.

Qualificações

• Completion of a Nursing Assistant or Patient Care Tech program.
• One year of experience working with a frail elderly population.
• In lieu of school verification, CNA certification may be accepted at time of hire.

Responsabilidades

• Provide participant care based on individualized plan of care with the Interdisciplinary Team.
• Document and report participant status and maintain confidential medical records.
• Assist with treatments, procedures and maintenance of patient care equipment.
