Holton Manor in Elkhorn, WI seeks a Certified Nursing Assistant (CNA) to provide patient care under licensed staff supervision. You will perform data collection, assist with care plans, and communicate with residents and families to ensure safe, timely care in a supportive environment.

The role offers on-site work with flexible scheduling, competitive hourly wages, and benefits across full-time, part-time, and PRN positions.

Qualifications

• Must be in good standing on the State Nursing Assistant registry or appropriate entity.
• Ability to meet physical and job-related requirements.
• Must be English literate with excellent written and verbal communication.

Responsibilities

• Performs initial and ongoing data collection on all patients/residents under RN/LPN supervision.
• Implements plan of care based on patient needs.
• Provides direct patient care and reports changes to RN or LPN.
• Demonstrates time management and prioritization of tasks.
• Documents results of patient care within policies and procedures.
• Supports nutritional care.
• Communicates effectively with residents and families.
• Completes all other duties as assigned.
