Job description

• Collaborate with the Interdisciplinary Team in delivering quality patient/resident care under the direction of a licensed nurse
• Deliver age-specific patient/resident care
• Contribute to overall unit functions
• Manage admissions, discharges, and transfers
• Prepare rooms and equipment for new admissions and subsequent admissions
• Greet and orient patients/residents and families to the facility and room
• Inventory and document patient belongings during admission, transfer, and discharge
• Safely and punctually transfer or transport patients to activities, therapies, and departments
• Ensure availability, cleaning, storage, and cost-effective use of patient equipment and supplies
• Complete delegated tasks within scope of practice in coordination with the licensed nurse
• Participate in implementing plans of care and established goals
• Complete routine hygiene, positioning, and other unit tasks
• Maintain patient privacy and confidentiality
• Complete accurate and timely documentation of patient care
• Identify and communicate patient/resident care concerns and condition changes
• Take rapid action in life‑threatening situations and immediately notify the licensed nurse
• Communicate out‑of‑scope requests, transfers, discharges, and incomplete work requiring follow‑up
• Comply with universal precautions, infection‑control policies, restraint‑management policies, and safety procedures
• Report personal injuries, hazards, and unsafe practices
• Prioritize workload according to patient needs and staffing patterns
• Coordinate with staff, seek assistance and feedback, and report workload constraints to the licensed nurse or supervisor

Requirements

• Completion of an approved nursing assistant course
• California Certified Nurse Assistant (CNA) certification from the California Department of Public Health — REQUIRED
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) certification from the American Heart Association — REQUIRED
• Current certification as Nursing Assistant for long‑term care and skilled nursing — required
• Rehab experience preferred
• 1 year recent experience in a geriatric setting or acute care hospital — preferred
• Effective interpersonal skills
• Ability to perform all baseline resident/patient care skills under guidance of a licensed nurse
• Fluent knowledge (written and spoken) of English
• Employees must join the union within 31 days of hire and remain a member or qualifying contributor for the duration of the collective bargaining agreement
