Job description

Position Purpose

To provide Direct Resident Care under supervision of the Charge Nurse. The Nurse Assistant follows established policies and procedures while providing and coordinating resident care with other facility personnel.

Performance Standards
  1. Follows established performance standards and performs duties according to nursing service policies and procedures.
  2. Requests clarification and/or training for policies and procedures that are not clearly understood.
  3. Assists new employees in following established facility policies and procedures.
  4. Identifies special resident problems and reports them immediately to Charge Nurse.
  5. Conducts Resident Care as assigned, completing assignments accurately and in a timely manner.
  6. Conducts Resident Rounds daily and initiates corrective action as necessary and/or seeks assistance of Charge Nurse, Treatment Nurse or Nursing Supervisor.
  7. Identifies safety hazards and emergency situations and initiates corrective action immediately.
  8. Provides nursing care to residents without violating Residents’ Rights.
  9. Participates in Facility Education Programs as assigned.
  10. Attend all classes as assigned and completes assignments accurately and in a timely manner.
  11. Demonstrates an attitude of cooperation and enthusiasm during instruction and in resident care activities.
  12. Assumes personal responsibility for following facility procedures related to control of equipment and supplies within the unit.
  13. Assume accountability for compliance to Federal, State and Local regulations within the unit assigned and within the span of control of the Nurse Assistant.
  14. Reports unusual problems or incidents to Charge Nurse or Supervisor.
  15. Accurately documents incidents and/or unusual problems according to established facility procedures.
  16. Demonstrates consistent ability to work cooperatively with residents, Charge Nurse, Treatment Nurses, Restorative Assistants, other Nurse Assistance, Physicians, families, consultant personnel, and ancillary service providers.
  17. Participates in Resident Care conferences and other facility meetings as assigned.
  18. Participates in the development of an individualized plan of resident care for residents assigned. Reviews Care Plan daily and performs nursing care as outlined.
  19. Provides nursing care as outlined by the charge Nurse and the resident Care Plan.

20. Documents in the Nurse Assistant notes each shift of duty regarding care and treatment provided to the resident and the resident’s response of lack of response to care provided.

21. Reports changes in residents’ conditions immediately to Charge Nurse or Nurse Supervisor.

22. Performs basic skills as outlined in facility educational program, demonstrating knowledge and competence.

23. Seeks assistance when confronted with a resident problem that requires special assessment.

24. Follows facility procedures for admission, discharge and transfer of residents and completes all documentation accurately and timely.

25. Greets residents by name and speaks to resident and family members in a kind, thoughtful manner.

26. Listens to resident and family complaints attentively and reports problems to Charge Nurse promptly.

27. Responds appropriately to the feelings of others consistently by listening attentively and taking follow up action.

28. Controls angry feelings appropriately.

29. Demonstrates concern for all residents’ welfare by rendering immediate assistance to any resident in need.

30. Answers call lights promptly.

31. Works with a cooperative spirit wherever assigned.

32. Demonstrates warm, caring feelings about residents by responding appropriately to the needs expressed.

33. Comes to work as scheduled and consistently demonstrates dependability.

34. Comes to work in clean, neat uniform and consistently adheres to dress code.

Job Tasks
  1. Clocks In, Receives report and assignment.
  2. Gives care to all residents assigned.
  3. Serves breakfast trays and assist with feeders.
  4. Removes all trays, pulls up rails and rolls beds down.
  5. Obtains necessary linen, soiled linen container and proceeds with assignment.
  6. Before starting baths, checks all residents, rolls down beds, changes incontinent residents and takes ambulatory residents to bathroom.
  7. Completes tub baths, showers or bed baths as scheduled.
  8. Pays special attention to oral hygiene and car of nails and hair.
  9. Makes sure all call lights are secured within reach of residents and all bed rails are up on the beds before leaving room.
  10. Reports immediately to Charge Nurse anything unusual about residents.
  11. Takes all residents to bathroom before placing in wheel chairs. (All wheel chair residents are to be checked frequently and taken to the bathroom at least every two hours)
  12. Turns and repositions all bed and chair residents every two hours.
  13. Places Bed pans, linens, washbasins and soiled linen in proper area.
  14. Keeps bedside and over-bed tables clean and uncluttered. Keeps side rails clean.
  15. Makes sure residents are up in wheelchairs for activities. (A.M)
  16. Takes residents to dining room and assists in feeding if assigned to dining room.
  17. Walks ambulatory residents to dining room and takes them to bathroom every two hours and as requested.
  18. Makes sure all residents have fresh water. Encourages fluids for all residents unable to take water unassisted.
  19. Makes sure all residents are offered nourishment at indicated times.
  20. Encourage adequate fluid intake by providing every resident with ice water.
  21. Makes sure food percentage, intake and output, clinitest/ acetest and bowel and bladder records are completed before going off duty.
  22. Checks name bands on all assigned residents every day. Reports to Charge Nurse if resident has removed name band or if name bands need to be changed.
  23. Checks all special assignments and sees that each assignment is completed before going off duty.
  24. Empties and records all urinary drainage units.
  25. Checks all residents at the of the shift and makes sure they are clean, dry, comfortable, have all call lights within reach, bed rails are up (if residents are in bed) and are safe.
  26. Charts on all residents, making sure to include a narrative note about the activity and progress of the resident that day. Signs all notes with first initial, last name and title.
  27. Report to CNA of on-coming shift.
  28. Other duties as assigned by the Charge Nurse, Supervisor and/or Director of Nursing Services.

Requirements
  • CNA licensed by State of CA
  • CPR & First-Aid Certification

  • Must clear criminal background with DOJ and FBI
  • Ability to work in a healthcare setting with the senior population
Physical Requirements of Position
  • Ability to lift fifty (50) pounds; Moving, lifting, assisting or transferring of patients may involve lifting of up to 100 pounds using proper mechanics and/or assistive lifting devices.
  • Ability to exert maximum muscle force to lift, push, pull or carry patients
  • Ability to use abdominal and lower back muscles over time without fatigue
