Job summary

Crossroads Hospice is seeking a dedicated CNA to provide compassionate bedside care in our hospice program. You will play a vital role within the interdisciplinary team, ensuring comfort and dignity for patients and support for families.

The position requires current CNA certification and at least 6 months of CNA experience, with preference for hospice/home health exposure. The role includes a weekday schedule with some travel to our Plymouth Meeting, PA office, and comprehensive benefits.

Qualifications

• Must have current, state CNA certification.
• 6 months of current experience as a CNA is required.
• Hospice or Home Health Agency experience preferred.
• Must possess interpersonal skills sufficient to interact with patients, families, and caregivers.
• Must learn computer skills related to required clinical software and electronic records.

Responsibilities

• Provide bedside care and support to patients in a hospice setting.
• Serve as the eyes and ears of the interdisciplinary team at the bedside.
• Assist with patient and family communication and comfort during care.
• Demonstrate ability to learn and use required clinical software.

CNA Qualifications

• Must have current, state CNA certification
• 6 months of current experience as a CNA is required
• Hospice or Home Health Agency experience preferred
• Must possess interpersonal skills sufficient to interact effectively with patients and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors
• Must exhibit or demonstrate the ability to learn computer skills related to required clinical software and electronic clinical record utilization
• Must have personal transportation, valid driver’s license, required automobile insurance, and the ability to safely and independently operate said vehicle

CNA Schedule & Benefits

• Schedule: Monday to Friday, 8:30 AM - 5:00 PM
• Travel to the office in Plymouth Meeting, PA is required
