Job summary

Crossroads Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our Even More Care program. You will support patients and families, respond to urgent needs, and work closely with an interdisciplinary team to provide dignity and comfort during end-of-life care.

The role emphasizes bedside support, teamwork, and compassionate presence for patients in the final stages, with opportunities for professional growth within the organization.

Qualifications

• High school diploma or G.E.D. required.
• Licensed as a state-certified nurse’s aide with an unrestricted, active aide certification.
• 3–6 months of hospice or home health aide experience preferred.
• Interpersonal skills to interact with patients, families, and the care team.
• Ability to learn clinical software and electronic medical records.

Responsibilities

• Care for patients and families as part of the interdisciplinary team.
• Respond to urgent physical or psychosocial needs in the Even More Care program.
• Provide attentive bedside presence when patients are in active dying stages.

CNA Qualifications

• High school diploma or G.E.D. required
• Must be certified by the state as a nurse’s aide with an unrestricted, active aide certification
• 3 to 6 months of current experience as a Hospice Aide or Nurse’s Aide required
• Hospice or Home Health Agency experience preferred
• Must possess interpersonal skills sufficient to interact effectively with patients and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors
• Must exhibit or demonstrate the ability to learn computer skills related to required clinical software and electronic clinical record utilization
• Must have personal transportation, valid driver’s license, required automobile insurance, and the ability to safely and independently operate said vehicle

CNA Schedule & Benefits

• Schedule: Wednesday, Thursday & Friday, 8:00 PM to 8:30 AM
• The office is located in Plymouth Meeting, PA and sees patients within a 50-mile radius.
