Brighton/Suncrest Hospice in Brookhaven, PA, is seeking a dedicated coordinator to support our CNA field staff, ensure complete daily schedules, and reassign patient visits as needed to maintain high-quality care.

You will assist with staffing assignments, respond to calls from families and referral sources, help orient new CNA staff, and compile weekly reports for review with your supervisor. Strong teamwork and communication are essential.

Competenze

• Ability to work well in a team and independently.
• Excellent communication skills with patients, families, and staff.
• Hospice experience is preferred but not required.
• Strong familiarity with computer applications and spreadsheets.
• Leadership experience is preferred.

• Receive callouts from CNA field staff and reassign patient visits as needed.
• Ensure daily patient schedules are complete and updated in the database; assist with staffing assignments.
• Respond to inquiries from families, referral sources, and other disciplines; provide support and education as needed.
• Assist with new-hire orientations for CNA staff.

Details

• Receive callouts from CNA field staff and reassign patient visits as needed
• Ensure that the daily schedule of patients is complete and updated in database. Assist with staffing assignments for new admissions and scheduling as needed/directed by supervisor.
• Respond to phone calls from all disciplines, family members and referral sources to assist with concerns or general CNA related questions. Follow up with staff to provide support and education as needed.
• Assist with the preparation and facilitation of new-hire orientations for all CNA field staff
• Complete weekly reports which will be addressed in a weekly meeting with direct supervisor
• Assist in reviewing and exporting all visits in our database, communicating with CNAs in reference to their documentation and timecards and then reporting this information to supervisor weekly.
• Assist in field as needed

Qualifications

• Ability to work well within a team environment but also able to work independently
• Excellent communication skills
• Prior hospice experience is preferred, but not required
• Strong familiarity with computer applications and spreadsheets
• Leadership experience preferred
