Job summary

Brighton Hospice is seeking a coordinating caregiver liaison to manage CNA field visits and daily patient scheduling. You will reassign visits, support staffing for admissions, and maintain accurate records under supervision.

The role emphasizes strong communication with families and staff, onboarding of new CNAs, and weekly reporting to improve patient care and operational efficiency. Hospice experience is preferred but not required.

Qualifications

Responsibilities

• Receive callouts from CNA field staff and reassign patient visits as needed.
• Ensure daily schedule of patients is complete and updated in the database.
• Assist with staffing assignments for new admissions and scheduling as directed by supervisor.
• Respond to inquiries from families, staff, and referrals; provide support and education as needed.
• Assist with new-hire orientations for CNA field staff.
• Prepare weekly reports and communicate findings to supervisor.

Details

• Receive callouts from CNA field staff and reassign patient visits as needed
• Ensure that the daily schedule of patients is complete and updated in database. Assist with staffing assignments for new admissions and scheduling as needed/directed by supervisor.
• Respond to phone calls from all disciplines, family members and referral sources to assist with concerns or general CNA related questions. Follow up with staff to provide support and education as needed.
• Assist with the preparation and facilitation of new-hire orientations for all CNA field staff
• Complete weekly reports which will be addressed in a weekly meeting with direct supervisor
• Assist in reviewing and exporting all visits in our database, communicating with CNAs in reference to their documentation and timecards and then reporting this information to supervisor weekly.
• Assist in field as needed

Qualifications

• Ability to work well within a team environment but also able to work independently
• Excellent communication skills
• Prior hospice experience is preferred, but not required
• Strong familiarity with computer applications and spreadsheets
• Leadership experience preferred
